After a standard transfer auction with South's one notrump showing 15-17 points and his three spade bid showing four card spade support you decide to lead the three of hearts.
This goes to partner's jack and declarer's ace.
Declarer draws trumps in two rounds ending in dummy and plays a diamond to the queen and king.

There is a danger that either declarer's hearts or clubs are going on the long diamonds in dummy, but has partner got the king of hearts or the ace of clubs?
Counting declarer's points solves this problem, he has shown up with fourteen points so far (SA, HA, DAQ) so cannot have the club ace, so switch to a club.
